Insurance Information

Our treatment coordinators deal with a variety of insurance companies, PPOs, HMOs, and some medical plans as well.  Insurance plans are riddled with clauses, policy and frequency limitations, so the more information you can provide us about your particular plan will help us provide the best benefit options for your treatment.   

Basic Information Needed

• Insurance company name and telephone number
• Group number
• ID or social security number depending on the insurance plan
• Dates of birth for patient and guarantor

Regarding A Traditional PPO

A PPO plan has a calendar year maximum.  Each time you go to the dentist, a part of your maximum is used.  So it is a good idea to know how much has been used of your maximum after seeing your general dentist. 

Regarding HMO/DMO Plans

These plans do not have a calendar year maximum.  In order for the specialist's office to bill, they must have a written referral by the general dentist office.  All treatments have to be pre-authorized with the exeption of a single-tooth emergency extraction.
