Our office hours are:

Monday: 8 am - 5 pm
Tuesday: 8 am - 5 pm
Wednesday: 8 am - 5 pm
Thursday: 8 am - 5 pm
Friday: 8 am - 5 pm

Please assist us at the time of your appointment/s to the office by providing the following information:

Your referral slip and x-rays from your referring dentist.
A list of medications you are presently taking (please note that you can fill out your patient registration and medical history form on this web page.)
Be sure to notify the office in advance if you have a medical condition or concern prior to surgery (e.g. artificial heart valves or joints, heart murmurs requiring premedication, severe diabetes, or hypertension.) Your medical doctor's information with regard to a complex medical history will be necessary for clearance prior to surgery.
If you have dental insurance, please contact the office 1 or 2 days in advance of your appointment so that we may have everything prepared on the day of your first visit.

Please Note:

All patients under the age of 18 must be accompanied by a parent/guardian for consultations & Surgery.
